1. Government Issued ID - Must be 18 years or older.
2. SSN (State and reporting requirement).
3. HS Diploma or Equivalency (Required no later than class 3). Foreign documents must have an “Evaluation” equivalent to a HS Diploma or higher (NOT a translation). Transcripts must show a graduation date (even college transcripts). NO Wonderlic allowed.
4. Immunizations or Declination.
5. Sign an Enrollment Packet, which includes state-required forms: Enrollment Agreement, CSC-005, CSC-010, Declination, and Tour Form.
